The RN will work to develop a Plan of Care that tailors to the Individual’s specific medical needs. Periodically, the RN will reassess the patient to monitor any changes with the patient’ health that may need to be addressed with the primary physician.
Speech Therapy (ST) works with patients who have difficulties with speech and swallowing. ST’s goal is to teach a patient how to communicate and eat unassisted.
Physical Therapy (PT) works with the patient to strengthen their ability to move, reduce pain, and restore function. PT’s goal is to teach the patient and family an at home regimen to aid in the recovery process.
A medical social worker (MSW) works with the patient to evaluate if the patient would qualify for additional services provided by the community. These services may include: provider services, meal preparation/delivery, pest extermination, etc.
Occupational Therapy (OT) works with a patient to progressively be able to complete activities of daily living (ADL) unassisted. OT’s goal is to strengthen the patient physically and emotionally by encouraging the patient to become more Independent.
Home Health Aides (CNA) works directly with the patient to ensure their hygienic needs are met. The home health aide will supervise bathing, help with grooming, and assist with getting dressed.
