Clinical Status:
- History of falls or injuries due
- to a fall in the last six months
- Require home therapy
evaluation - Symptom management issues
such as weakness, confusion - Vision problems
- Balance or unsteady gait
- Difficulty ambulating with or
without a device
Functional Status:
- Able to communicate via telephone
- Willing and able to perform a
home exercise program
What we will do for our patients:
- Assess patient for all risk factors
- Home Safety Assessment
- Assess patient knowledge deficits
- Develop individualized plan of care
- Physical Therapy
- Anodyne Therapy (for patients with
pain, numbness or Neuropathy) - Instruct patient on medications
- Instruct patient on exercise program
- Instruct patient on nutrition/hydration
- Instruct patient on safe transfers/
mobility - Instruct patient on proper use of
assistive devices - Provide educational material
